Job Summary

Reporting to the Supervisor, Healthy Babies Healthy Children, this position is responsible for the delivery of the Healthy Babies Healthy Children program within the Healthy Families Division.

Duties & Responsibilities

The successful candidate will use principles of health promotion, build individual and community capacity, establish and maintain relationships, advocate for access and equity and demonstrate professional responsibility and accountability in the following assignments:

  • Provide assessment, health promotion and health education to promote and support infant and parental mental health, breastfeeding, positive parenting, child growth and development and injury prevention.
  • Provide case management, service coordination, counseling and referrals for families impacted by complex challenges.
  • Remain current with Public Health Program Standards, HBHC protocols and guidance documents, resources available to clients and departmental and program policies and procedures.
  • Collaborate with key stakeholders such as public health program staff, Social and Community Services staff and hospital and community partners to plan, implement and evaluate programs and/or projects.
  • Contribute as a team member; Provide Health Department representation on community-based coalitions and groups as required.
  • Provide excellent customer service.
  • Maintain accurate health records and statistical information.
  • Provide input into and implement quality assurance activities.
  • Perform other duties as assigned.

Skills & Qualifications

  • The successful candidate must possess a Bachelor of Science in Nursing from a recognized University and hold a current Certificate of Registration in good standing from the College of Nurses of Ontario.
  • The ideal candidate will have recent experience working in the Healthy Babies Healthy Children home visiting program, including Parent Child Relationship Program (NCAST)certification and experience documenting within the CDIS (formerly HCD-ISCIS) data base.
  • Breastfeeding Management Certification is required.
  • Candidates must be knowledgeable about positive parenting, child growth and development and Trauma and Violence Informed Care.
  • Candidates must possess excellent communication and motivational interviewing skills, have proven ability to develop therapeutic relationships with families impacted by complex challenges and demonstrate the ability to work collaboratively using a service coordination approach.
  • Strong project management skills and computer software proficiency are fundamental.

Additional Skills and Qualifications

  • Knowledge of local services and resources within Halton Region is considered and asset.
  • Level 2 Breastfeeding Management is preferred. 

Working/ Employment Conditions

Working conditions

  • This position may involve work in office, remote work from home, and in-person service delivery in various settings.
  • Flexibility in work location is required to meet operational needs, and extensive daily travel may be required between locations within Halton Region. Incumbent must supply own transportation.
  • Flexibility to work outside of core business hours including evenings, weekends and holidays is also required.
  • The incumbent must be able to provide a current complete record of immunization according to the Ontario provincial immunization schedule.

  • The successful candidate will be made an offer of employment on condition that the Regional Municipality of Halton receives a current (obtained within the past six (6) months), original and acceptable Vulnerable Sector Check by the first day of employment.
