Job Summary

Reporting to the Supervisor of Community Support Services and Coordinators for the day to day operations, staff will participate as a member of the Adult Day Program team, supporting all aspects of client care based on the client’s care plan. This includes supporting the client with activities of daily living, restorative programs, health and wellness promotion and with therapeutic recreation programs.

Duties & Responsibilities

  • Adhere to regulations under the Personal Health Information Protection Act (PHIPA) and other applicable Regional policies and protocols that dictate the expectations for the handling of confidential information.  
  • Assist clients in performing activities of daily living including toileting, feeding, lifting and transferring and mobility and other activities according to clients’ individual care plan.
  • Ensure that clients’ needs are met during the day.
  • Assist with the preparation and delivery of meals, snacks.
  • Assist in the provision of a bathing program.
    Assist with the provision of therapeutic recreation activities, including one to one programs and interventions (i.e. Montessori activities) and group programs.
  • Observe reports and document changes in the client’s status and contributes to the development of client care plans in Point Click Care.
  • Develop and maintain all client support forms such as personal care schedule.
  • Assist with cleaning duties as assigned such as clean-up of program areas, laundry and meal area.
  • Communicate effectively with clients, staff and families.
  • Provide excellent client/customer service at all times, greeting all clients, caregivers and visitors.
  • Function as part of an effective team, supporting team members, volunteers and students in all programs and duties.
  • Participate in internal (i.e. weekly client review) or external committees and meetings.

Skills & Qualifications

  • Personal support worker certificate from a recognized college.
  • One (1) to two (2) years’ experience in a community setting such as an adult day program or long-term care.
  • First Aid and CPR certification.
  • Experience in working with those with dementia, responsive behaviours and older adults with physical disabilities.
  • Ability to be flexible, patient, and tolerable.
  • Dependable, reliable, and punctual.
  • Have an understanding of the Client’s Bill of Rights.
  • Good oral and written skills.
  • Knowledge of computer programs including MS Office and Point Click Care an asset.
  • Additional training including Food Handlers Certificate, Gentle Persuasive Approach (GPA), and Montessori Methods are considered assets.

Working Conditions

  • Physical demands include lifting, pushing and pulling.
  • Flexibility in work schedule for occasional evening and weekend work as needed.
  • This position requires availability and willingness to work rotating shifts at both Milton Place Adult Day Program (Milton) and Silver Creek Place Adult Day Program (Georgetown). Flexibility to work varying shifts on short notice is required.

 

Employment Conditions

  • Current (obtained within the past six (6) months), original and acceptable Vulnerable Sector Check by the first day of employment.
  • Current (obtained within the past six (6) months) two-step tuberculosis test or Quanti-FERON-TB Gold plus test by the first day of employment.
